QR Codes linked to child's vaccination record

About 3 million children die every year because they don't have access to vaccines that prevent diseases. Immune Project is looking to lower those numbers or at least manage patient records in a better way.
What the application does is that it is linked with a QR Code that can be worn around children's neck, or on an ID, kept with the mother or it can be embedded in child's toys.
When the QR Code is scanned, medical workers will be able to pull up the vaccination record and can determine what sort of vaccines will be needed.
Medical worker will be able to prepare themselves for vaccine and the medication that they need to bring and so it will save them multiple trips and time that could cost a child's life if they don't make it back in time.
